Indications/Uses
Adult: Lower resp tract infections (including pneumonia & bronchitis), complicated & uncomplicated UTI (including pyelonephritis), skin & skin structure infections, intra-abdominal infections (including peritonitis & biliary tract infections), gynecologic infections, septicemia. Empiric treatment of febrile neutropenia. Childn: Pneumonia, complicated & uncomplicated UTI (including pyelonephritis), skin & skin structure infections, septicemia. Empiric treatment in febrile neutropenia.
Dosage/Direction for Use
Adult 1 g IV/IM every 12 hr. Duration of therapy: 7-10 days but may be longer in severe infections; 7 days or until resolved for empirical treatment of febrile neutropenia. Max dose: 2 g every 8 hr. Mild to moderate UTI 500 mg to 1 g IV/IM every 12 hr. Mild to moderate infections other than UTI 1 g IV/IM every 12 hr. Severe infections 2 g IV every 12 hr. Very severe or life-threatening infections 2 g IV every 8 hr. Childn >2 mth ≤40 kg Pneumonia, UTI, skin & skin structure infections 50 mg/kg every 12 hr for 10 days, may increase up to every 8 hr for more severe infections. Septicemia & empiric treatment of febrile neutropenia 50 mg/kg every 8 hr for 7-10 days. Childn 1-2 mth 30 mg/kg every 12 hr or 8 hr. Childn >40 kg Adult dosing. Do not exceed max adult dose for all childn doses.
Contraindications
Hypersensitivity to cefepime, cephalosporins, penicillin & other β-lactams.
Special Precautions
Diarrhea associated to antibiotic use. Overgrowth of nonsusceptible organisms. Renal impairment. Pregnancy & lactation.
Adverse Reactions
Rash, pruritus, urticaria; nausea, vomiting, oral moniliasis, diarrhea, colitis; headache; fever, vaginitis, erythema.
Drug Interactions
High plasma conc w/ probenecid. Drugs w/ nephrotoxic potential eg, aminoglycoside, potent diuretics.
